Your suggestions are needed for the Community Transformation Initiative, a joint Houston and Harris County project led by HDHHS.Community transformation is a movement across the country that seeks to improve the conditions where people live, work and play to help them make healthier choices. Community input is an important piece of the transformation movement. Your participation is appreciated no later than Friday, August 31, 2012, by completing a 12-minute online survey available in English and Spanish at www.gatewaytocare.org/survey.The goal of the initiative is to create healthier communities by making it easier for people to live a healthy lifestyle.
